Should I keep my child at home?
This is a popular question, although not always easy to answer. We are always available by phone or email to answer any questions; please do not hesitate to reach out.
A child who is sick will not be able to perform well in school and is likely to spread the illness to other children and staff. We suggest planning for childcare ahead of time so you will not be caught without a comforting place for your child to stay if he/she is ill. Remember- healthy children learn best.
Physical Exam Requirements
Physical exams are required for all students in grades Pre-K/Kindergarten, 1, 3, 5, 7, and 11. New Students are required to provide immunizations and physicals in accordance with New York State Health Department standards. The physical exam must be dated within 12 months prior to the start of the school year, or the date of entrance to the school for new entrants. If a copy of the health examination is not on file within 30 days, you will be notified by the health office. Please note that a current physical is also required for those who wish to participate in sports as well as those requesting working papers.
I recommend that your child’s physical examination be made by your family health care provider who knows your child best. During the doctor visit please check with your child’s physician if your child is due for any mandatory grade level immunizations. The New York State Dept. of Health physical form can be found on this page. Completed Dental forms are optional, but strongly recommended. The Dental health certificate form can also be found on this page.
Please return the completed physical and optional dental form to the health office or ask the doctor’s office to fax it directly to the Health Office at (716) 942-3440.
If your child has a physical scheduled during the school year, please notify the Health Office with the date of the scheduled physical exam. If you have any questions, please call the Health Office.
Immunizations
Medications in School
Both New York State and WVCS policy require parental and physician written consent for the administration of medication during school hours. This includes over the counter medication like Tylenol and Ibuprofen. Medication must be provided to the school nurse in the original container by the parent/guardian. Written consent is good for one school year. NY State law and school policy also require physician and parental written consent for students who self-administer inhalers, epi-pens or insulin.

Athletics Health Infomation
Athletic Placement
The Athletic Placement Process is for 7th and 8th grade students who want to play a JV or Varsity sport. State regulations state that the student must go through this process before being able to play at the higher level. Please see Coach Hansen or me to express your interest and to receive a form to fill out. Once the form is filled out and the parent permission slip has been signed, then the athlete must be cleared. Once the athlete has been cleared, then they may start the physical fitness testing with the gym teacher. The student must be cleared and pass the physical fitness test before they can participate in JV or Varsity practice or games. Please be sure to Coach Hansen as early as possible if you are a 7th or 8th grader and want to play JV or Varsity because this can be a long process.
Other Sports requirements
- Students who have been injured or had an extended illness or absence since their last physical exam will need updated physician clearance for sports participation.
- The school nurse must sign the Interval Health History forms before a student is allowed to practice or play. If the coach does not have a copy of the form signed by both the parent/guardian AND school nurse, the student may not participate in practice or play. The school nurse will not sign the Interval Health History form unless all information is up to date and student is completely cleared by physician.
- It is recommended that students use their own private health care provider for physicals (a separate “sports physical” is no longer required). If you are unable to have a physical performed by your private provider, the school medical director can provide a physical for your child. Please alert the school nurse if your child needs a physical performed at school.
