New Student Registration

  • This page contains the necessary forms for registering new students who reside within our district.

    (Please note: we may accept non-resident out of district students. A family interested in inquiring about this application process can contact the Superintendent’s Office at 716-942-3100 ext. 4102.)

    Once you've completed and obtained all the necessary registration documents, please contact Mrs. Westfall to set up an appointment.

    Please bring the following with you to your registration appointment:

    • Completed registration forms (see below to download the forms)
    • Proof of residency*
    • Birth certificate/proof of age for child(ren)

    *Ways to show proof of residency:

    • Lease or deed
    • An affidavit (a written statement signed under oath) from the person you pay rent to saying you live there
    • Pay stub showing your address
    • Income tax form that shows your address
    • Utility bill in your name
    • Membership document based on residency, such as a local library card
    • Voter registration card
    • Driver’s license, permit or non-driver ID
    • State or other government issued ID
